Abstract Purpose:Thyroid ophthalmopathy sometimes develops into thyroid optic neuropathy, causing severe visual acuity and visual field disturbances. In most cases, remission is achieved with pulsed steroid therapy, but in some cases, treatment resistance may lead to a need for orbital decompression or radiotherapy. In this report, we describe two cases of thyroid optic neuropathy refractory to steroid therapy that responded to orbital decompression and radiotherapy more than 6 months after treatment.

Cases:Case 1 was a 68-year-old man and Case 2 was a 59-year-old woman. Both had thyroid ophthalmopathy caused by Graves' disease and were referred to our hospital for steroid pulse therapy because of worsening symptoms of diplopia and ocular motility disorder.

Findings:Case 1:At the initial visit, visual acuity was(1.0)on the right and(1.2)on the left. Two courses of steroid pulse therapy were administered, but two months later, visual acuity decreased to(0.4)on the right and(0.3)on the left. Five months after orbital decompression, visual acuity had not improved, so irradiation was performed. Visual acuity recovered around 8 months after irradiation.

 Case 2:Initially, visual acuity was(1.2)on the right and(0.5)on the left. Two courses of steroid pulse therapy were administered, but two months later, visual acuity rapidly decreased to(0.15)on the right and(0.02)on the left, and the patient underwent orbital decompression surgery. Subsequently, neither visual acuity nor the visual field improved, so irradiation was performed. One year later, the peripheral visual field had recovered, but visual acuity had not.

Conclusion:In some patients, recovery of visual acuity and the visual field after orbital decompression and radiotherapy for steroid-resistant thyroid optic neuropathy may take several months.
