Abstract Purpose:To report a case of ophthalmic zoster sine herpete with pseudodendrite following topical tacrolimus treatment.

Case:A 46-year-old woman. Her chief complaint of blurred vision in her left eye.

 At the first visit, the cornea had superficial punctate keratitis like the Thygeson's superficial punctate keratitis, and her serum herpes viruses antibody titers(complement fixation)for differential diagnosis were less than 4 for herpes simplex virus, and 16 for varicella zoster virus(VZV). At 7 days after topical tacrolimus therapy, elevated pseudodendrites without terminal bulbs indicative of VZV sine herpete were observed. Treatment with topical aciclovir and polyvinyl alchohol iodide(PA iodide)5 times a day was started. Iridocyclitis was detected and 3.0 g oral valacyclovir a day was started. After the disappearance of the pseudodendrites at 13 days, topical 0.1% fluorometholone 6 times a day was started for lasting corneal edema and iritis;a clear cornea was observed after treatment.

Conclusions:Tacrolimus may have induced the development of pseudodendrites in the patient's immunocompromised cornea lymphocyte proliferation and modifying the local corneal immunity. Patients should be carefully followed-up after treatment with tacrolimus for any clinical adverse event.
