Abstract Purpose:To report a case of interdigitation zone(IZ)elongation after treatment for night blindness due to vitamin A deficiency.

Case:A 41-year-old man developed night blindness. He had corrected visual acuity of 1.0 in both eyes. An electroretinogram(ERG)showed severe rod dysfunction. Optical coherence tomography(OCT)revealed irregularity in the IZ with IZ lengths of 1,172 μm in the right eye and 1,542 μm in the left eye. The serum vitamin A level was of 64 IU/dL. These findings led to the diagnosis of vitamin A deficiency. One month after peroral vitamin A administration with a daily dose of 5,000 units, the ERG indicated improved and night blindness disappeared. OCT showed a well-defined IZ and elongation of the IZ length to 2,112 μm in the right eye and 2,586 μm in the left eye. Three months after the start of treatment, the serum vitamin A level was 302 IU/dL, and IZ length remained improved to 2,839 μm in the right eye and 2,534 μm in the left eye.

Conclusion:Assessing the IZ using OCT may help to confirm diagnosis and monitor response to treatment.
