Abstract Purpose:We report a case of poor vision after insertion of a low addition segmental IOL(Lentis Comfort®:Santen), in which anterior capsule contraction was observed, and radial anterior capsule incision using Nd-YAG improved vision.

Case:An 81-year-old woman was referred for cataract surgery in 2021. She was mildly myopic with a visual acuity of-3.0 D in both eyes and no severe astigmatism or higher order aberrations. She had no ocular diseases other than cataracts. Lentis Comfort® was inserted in both eyes. Visual acuity at 1 week postoperatively was 0.9(1.0×−0.50D)in the right eye and 0.6(0.7p×cyl−1.00D)in the left eye, and at 2 months postoperatively was 1.5p(n. c.)in the right eye and 0.6(0.7×cyl−1.25D 80°)in the left eye. In short, uncorrected and corrected visual acuity in the left eye were poor. Mydriatic examination revealed an anterior capsule constriction only in the left eye, and anterior segment OCT showed that the decentration value, an indicator of IOL decentration was 0.14 mm in the right eye and 0.27 mm in the left eye, which were within normal range. We hypothesized that increased intraocular aberrations due to anterior capsule constriction may contribute to poor postoperative vision. The visual acuity improved to 1.0(1.2×cyl−0.75D 80°)after Nd-YAG laser radial anterior capsule incision, and the decentration value decreased to 0.20 mm.

Findings:In cases of poor postoperative vision in Lentis Comfort®-inserted eyes with anterior capsule constriction, radial anterior capsule incision with an Nd-YAG laser may decrease intraocular aberrations, and thus improve visual acuity.
