Abstract Purpose:To report a case of linezolid(LZD)-induced optic neuropathy during treatment for purulent spondylitis.

Case:A 59-year-old man presented with purulent spondylitis due to surgery for lumber disc herniation. Methicillin-resistant staphylococci was detected in surgical specimens, and LZD treatment started. He presented with gradually impaired vision in both eyes and came to our hospital. The best-corrected visual acuities(BCVAs)were 0.01(OD)and 0.04(OS). A routine ocular examination revealed only increased circumpapillary retinal nerve fiber thickness in both eyes using optical coherence tomography. Goldmann's perimeter revealed severe visual field defects in both eyes. No significant disorders were detected by MRI. We suspected LZD-induced optic neuropathy and discontinued the LZD medication. The BCVAs recovered 0.5(OD), and 1.25(OS), and visual field defects were also improved after 10 months discontinuation of LZD.

Conclusion:Patients should be carefully monitored during LZD medication.
