Abstract Purpose:To characterize authorship gender imbalance in major general ophthalmology journals from 2006 to 2020 with reference to geographic variation.

Methods:We used PubMed to retrieve original articles published in Ophthalmology, JAMA Ophthalmology, American Journal of Ophthalmology, and British Journal of Ophthalmology. The gender of the author was determined by using name identifier software and internet search.

Results:A total of 14,797 articles were eligible for this study, in which 36.2% and 25.5% of the articles had female first and last authors, respectively. Female authorship showed a significant increasing trend over time. As compared to the number of female authors from the USA, a significantly higher proportion of female authors was found in Australia, India and Singapore, a significantly lower proportion in Italy and Japan, which was comparable with that from the UK, Germany and France.

Conclusions:This study provided novel information about a significant geographic variation in the gender gap in authorship in ophthalmology.
