Abstract Purpose:To report a case of acute retinal necrosis(ARN)with visual impairment due to silicone oil(SO).

Case:A 28-year-old male patient

Findings:The patient was referred to our hospital for decreased visual acuity in the right eye. Best-corrected visual acuity(BCVA)was 0.2 in the right eye and 1.5 in the left eye at the time of initial examination. The right eye showed inflammation in the anterior chamber and ciliary injection. The fundus showed vitreous opacity and a prominent white exudative spot was seen in the periretinal area. Suspecting endogenous endophthalmitis or ARN, the patient underwent vitrectomy with SO tamponade because multiple tears and necrosis of the surrounding retina were observed. Anterior chamber aqueous polymerase chain reaction test was performed, and ARN of HSV type 1 was diagnosed. After systemic treatment with acyclovir and prednisolone, his BCVA in the right eye improved to 0.8 at 70 days postoperatively, but suddenly decreased to 0.09 with central scotoma at 4 months postoperatively. Optical coherence tomography revealed a loss of the nerve fiber layer and marked thinning of the ganglion cell layer and inner retinal layer in the paracentral fossa of the right eye compared to the left eye suggesting visual loss due to retinal toxicity of SO. Despite SO removal 6 months after surgery, SO retinal toxicity was irreversible, and the visual acuity ultimately remained at 0.07.

Conclusion:Although SO toxicity is a rare complication, it is potentially irreversible and should be carefully monitored.
