Abstract Purpose:To report a rare case of binocular endogenous endophthalmitis due to Listeria monocytogenes.

Case:An 84-year-old man presented with bilateral severe iritis with dense vitreous haze, one week after pacemaker insertion. He had been receiving oral prednisolone and azathioprine for ulcerative colitis. Vitrectomy was performed immediately in both eyes. Culture of the vitreous humor showed Listeria monocytogenes leading to the diagnosis of infectious endophthalmitis. The retina of the right eye became detached, and vitrectomy was performed twice with scleral buckling;however retinal detachment recurred, and the final visual acuity was hand motion. The left eye's corrected visual acuity improved to 0.15.

Conclusions:Listeria monocytogenes should be considered as a causative agent of endogenous endophthalmitis in higher risk patients, such as immunosuppressants and elderly people.
