Abstract Purpose:We report a case of infectious corneal ulcer whose culture results were negative. The microscopic study obtained by corneal scraping was helpful for diagnosis and ocular manifestations of its treatment as a fungal keratitis.

Case Presentation:A 15-year-old female who frequently used replacement soft contact lens(SCL)presented with impaired vision in the right eye, conjunctival hyperemia, ocular pain, lacrimation and photophobia. Her visual acuity during the first visit was 0.02 in the Snellen scale in the right eye and corneal ulcer with opacity and infiltration was observed. Although corneal smear culture failed to detect pathogens, filamentous fungi were detected from a corneal scraping smear sample. This information was useful to plan the initiated three-combination treatment, and she entered our hospital to have her fungal keratitis treated. Topical and systemic antifungal drugs were continued for 2 months up until the corneal epithelium healed. Visual acuity improved to 0.7 at five months and 0.8 at 18 months.

Conclusion:If the result of culture obtained by corneal scraping is negative, the microscopic study is helpful in the diagnosis of infectious keratitis and the treatment of its ocular manifestations. Changes in the use of SCL without learning and mastering lens care should be avoided.
