Abstract Purpose:To report a case of hypophysitis in a pregnant woman with preeclampsia.

Case:A 35-year-old woman was referred to our hospital for preeclampsia. She received cesarean section at 30th week of pregnancy. She was referred to us one week later for headache and blurred vision both eyes that had been present since 25th week of pregnancy.

Findings and Clinical Course:Best corrected visual acuity was 1.2 right and 0.9 left. Both eyes showed hemorrhages and soft exudates in the fundus. Atypical temporal hemianopia was present in both eyes. Magnetic resonance imaging showed enlarged and homogenous enhanced mass in the pituitary fossa with suprasellar extension. Basal pituitary hormone levels were normal. Normal response resulted after exogenous administration of hypothalamic-releasing hormones. Hypertonic saline and desmopressin test showed partial central diabetes insipidus pattern. The findings were suggestive of hypophysitis, or infundibulo-neurohypophysitis. After treatment with 30 mg/day of prednisolone, visual acuity in the left eye improved to 1.2 one month later together with decreased pituitary enlargement. She has been doing well for 12 months until present.

Conclusion:This case shows that infundibulo-neurohypophysitis occurred in late stage of pregnancy with preeclampsia.
