外眼筋肥厚を伴う先天性固定斜視の1例

A case of congenital esotropia fixus with hypertrophic extraocular muscles

要約　目的：先天性の外眼筋肥厚が原因と考えられる重度の固定内斜視の小児の1例を報告する。

症例：3か月，男児。生後から右眼の眼球後退を伴う固定内斜視であった。右眼の眼球運動は全方向で強く制限されていた。

経過：活動性の炎症や腫瘍性病変は否定的であったため，生後5か月時に右内直筋と下直筋の切腱を施行した。術後，眼位は著しく改善した。

結論：外眼筋肥厚が原因と考えられる先天性の重篤な固定内斜視の症例に右内直筋と下直筋切腱を施行して眼位の改善が得られた。

Abstract　Purpose：To report a case of severe congenital esotropia fixus with hypertrophic extraocular muscles.

Case：A 3-month-old male infant presented with congenital enophthalmos and severe esotropia fixus of the right eye. Movement of the right eye was restricted in all directions.

Findings and Clinical Course：Clinical findings ruled out active inflammation or neoplasm. Magnetic resonance imaging（MRI）showed hypertrophic extraocular muscles of the right eye. The right eye received tenotomy of the medial and inferior muscle when the patient was 5 months old. The eye position markedly improved after surgery.

Conclusion：Tenotomy of medical inferior rectus muscles was effective for severe congenital strabismus fixus with unilateral hypertrophic extraocular muscles.
