内斜視による弱視治療中に脳腫瘍が発見された1歳女児の1例

Brain tumor in a one-year-old female infant with esotropia and amblyopia as initial manifestations

　1歳女児が1か月前からの眼位異常で受診した。右眼優位の内斜視があり，左眼固視が不良であった。外転制限はなく，他の神経症状もなかった。斜視弱視として治療を行ったが効果は乏しかった。4か月後に左眼乳頭が蒼白化し，視覚誘発電位検査（VEP）で左眼視神経障害が疑われ，CT検査で頭蓋咽頭腫が発見された。手術を行い，2歳6か月の現在，経過は良好である。脳腫瘍が斜視として初発することがあることを示す症例である。

　A one-year-old female infant presented with abnormal eye position. She had comitant esotropia fixating with the right eye. Otherwise,she was free of abnormal eye movements. Treatment for amblyopia was futile. Four months later,optic nerve disorder was suspected because of pale optic disc and abnormal visual evoked potential（VEP）. Further examinations led to the detection of craniopharyngioma. After surgery,she is doing well until the age of 2 years 6months. This case illustrates that brain tumor in an infant may manifest as strabismus.
