Abstract Purpose:To report the status and effect of ripasudil ophthalmic solution for glaucoma.

Cases and Method:This retrospective study was made on 82 eyes of 54 cases who were treated by ripasudil ophthalmic solution during 18 months through July 2016. The series comprised 43 male and 39 female eyes. The age ranged from 27 to 91 years, average 64.6±15.2 years. There were 29 eyes with primary open-angle glaucoma(POAG), 18 eyes with normal-tension glaucoma, 8 eyes with steroid glaucoma, and 7 eyes with other types of glaucoma. Intraocular pressure(IOP)before treatment with ripasudil ranged from 9 to 31 mmHg, average 17.5±0.5 mmHg. Medication score averaged 3.5±1.0. Treatment was evaluated as effective when IOP decreased by 10% or more at 3 months of treatment.

Results:A total of 67 eyes continued treatment with ripasudil 3 months after start, of which 38 were evaluated as effective and 29 eyes were ineffective. Lower age was significantly associated with effective response(p=0.013). Treatment was more effective for POAG and steroid glaucoma than capsular glaucoma. IOP averaged 16.6 mmHg before treatment, 15.0 mmlig at 1 month, 14.5 mmHg at 3 months, and 14.2 mmHg at 6 months. Treatment was discontinued in 12 eyes, of which 10 discontinued after 6 months of treatment.

Conclusion:IOP decreased even in eyes receiving multiple medications. IOP showed step-wise decrease during 6 months of treatment with ripasudil. Young patients showed more response. Side effects tended to develop after 6 months of treatment.
