Abstract Purpose:To report a case of extensive infectious corneal ulcer treated by standard with additional deep anterior lamellar keratoplasty(DALK).

Case:A 40-year-old male was referred to us for corneal ulcer in the right eye. Ocular pain had developed 25 days before and been treated under the diagnosis of corneal ulcer. He had started wearing soft contact lens for myopia one month before.

Findings and Clinical Course:Corrected visual acuity was 0.4 right and 1.5 left. The right cornea showed ulcer in the superior nasal quadrant. The anterior chamber showed signs of iritis. The corneal lesion exacerbated and started showing undermining. The right eye received Brown surgery by excision of conjunctiva adjacent to the limbus on day 66. Due to further exacerbation, the right eye received standard DALK with additional crescent-shaped one and Brown surgery on day 88. The cornea started to regenerate 6 days later and the epithelial defect disappeared 6 weeks later. The cornea has remained transparent with visual acuity of 0.9 for 2 years until present.

Conclusion:Modified DALK was effective for extensive infectious corneal ulcer in the present case. DALK enables complete removal of the involved cornea with minimum complications.
