Abstract Background:Parinaud's oculoglandular syndrome manifests as acute unilateral granulomatous conjunctivitis with ipsilateral regional lymphadenopathy. We present a case of Parinaud's oculoglandular syndrome caused by cat-scratch disease(CSD).

Case:A 45-year-old woman presented with conjunctival congestion and discharge in her left eye and painful swelling of the left parotid region. At initial presentation, a slit-lamp examination showed hyperemia and granuloma in her left inferior palpebral conjunctiva. Painful swelling of the left parotid region and a scratch mark on her left hand were also observed. Blood examination results showed increased C-reactive protein(CRP). Swelling of the left parotid region, and of the deep cervical, accessory, and supraclavicular lymph nodes were observed by computed tomography. She reported contact with pet cats. We prescribed oral azithromycin and bromfenac sodium eye drop. After treatment, her CRP level returned to normal level within 2 weeks and lymphadenopathy settled down in a week. Conjunctival granuloma healed in 6 months. Serum IgG tilter for Bartonella henselae was 1:256 at initial visit and had increased to>1:1,024 one month later;we finally diagnosed her condition as Parinaud's oculoglandular syndrome caused by CSD.

Conclusions:In case of granulomatous conjunctivitis with lymphadenopathy, CSD should be suspected and a testing for serum B. henselae antibodies is useful.
