Abstract Purpose: To report a case of central serous chorioretinopathy(CSC)that improved after peroral eplerenone for idiopathic aldosteronism.

Case: A 64-year-old male was referred to us for CSC in the right eye. He had been diagnosed with CSC in the left eye 15 years before. CSC in the left eye had shown repeated recurrences. He had been diagnosed with systemic hypertension 14 years before followed by treatment.

Findings and Clinical Course: Corrected visual acuity was 0.8 right and 0.3 left. Both eyes showed findings compatible with CSC. Hematological findings were suggestive of idiopathic aldosteronism. Further systemic studies showed secretion of aldosterone from both suprarenal glands. He was finally diagnosed with asymptomatic Cushing syndrome. He was treated by peroral eplerenone for hypertension. CSC in both eyes started to regress 3 weeks after start of treatment. There has been no recurrence for 4 months until present.

Conclusion: Peroral epleprenone was followed by regression of CSC associated with idiopathic aldosteronism.
