Abstract Purpose: To report the methods and outcome of treatment for nontraumatic corneal perforation.

Cases and Method: This retrospective study was made on 29 eyes of 27 cases who were treated for nontraumatic corneal perforation in the past 8 years. The series comprised 19 males and 8 females. The age ranged from 46 to 98 years, average 70 years. Cases were evaluated regarding causes of corneal perforation, method of treatment, visual outcome, and complications.

Results: Corneal perforation was secondary to infection in 17 eyes and not to infection in 11 eyes. The cause was not identified in one eye. Lamellar keratoplasty was performed on 14 eyes, penetrating keratoplasty on 7 eyes, enucleation in 2 eyes, and nonsurgical means in 6 eyes. Four eyes were treated by amnionic transplantation but all the cases needed additional surgery. Visual acuity improved in 6 out of 7 eyes after penetrating keratoplasty. Complications included graft infection, graft rejection, and ocular hypertension, and subsided after treatment.

Conclusion: Lamellar or penetrating keratoplasty was useful in closure of wounds in nontraumatic corneal perforation. Visual acuity improved in 6 out of 7 eyes that received penetrating keratoplasty.
