Abstract Purpose: To report a case of cytomegalovirus(CMV)retinitis that developed during treatment for systemic sclederma.

Case: A 75-year-old male was referred to us under tentative diagnosis of acute retinal necrosis. He had been diagnosed with systemic sclerosis with interstitial pneumonia and been receiving immunosuppresive treatment. He had received cataract surgery in both eyes.

Finding and Clinical Course: Best-corrected visual acuity was 1.2 right and 1.0 left. Both eyes showed retinal vasculitis and yellowish opacity in the peripheral retina. Aqueous humor from the left eye showed CMV-DNA processed by polymerase chain reaction. He was diagnosed with CMV retinitis and started treatment with systemic and vitreal injection of ganciclovir. Immunosuppressive treatment was continued as before. CMV level in the blood became negative 4 weeks later. Retinal exudates turned into scarred tissue after 7 weeks in the right eye and 11 weeks in the left. There has been no recurrence for 30 months until present with visual acuity maintained at 1.2 in either eye.

Conclusion: This case illustrates that CMV retinitis may develop following systemic corticosteroid or immunosuppressive agent and that it may not necessary be associated with human immunodeficienciency virus(HIV)infection.
