幼児における先天鼻涙管閉塞の涙道内視鏡所見

Endoscopic findings of congenital nasolacrimal duct obstruction in infants

要約　目的：先天鼻涙管閉塞（CNLDO）に涙道内視鏡を用いてプロービングを行った幼児の涙道内視鏡所見の報告。

対象と方法：CNLDOのため，全身麻酔下で涙道内視鏡によるプロービングを施行された7例8側。プロービングの年齢は12〜36か月であった。手術時の涙道内視鏡映像と診療録をretrospectiveに調べた。

結果：8側すべてで，涙囊と鼻涙管内に白色膿の貯留があった。涙囊内壁は6側で，鼻涙管内壁は全側がこぶ状を呈していた。鼻涙管尾部の閉塞部は1側のみがスリット形状で簡単に穿破できたが，それ以外は抵抗性があった。

結論：生後1歳を過ぎたCNLDOの涙囊と鼻涙管内には膿が貯留し，その内壁はこぶ状で，閉塞部も抵抗性があり，自然開口しにくくなっていると思われた。

Abstract　Purpose: To report endoscopic findings of the lacrimal sac and nasolacrimal duct in infants with congenital nasolacrimal duct obstruction.

Cases and Method: This study was made on 8 sites of 7 cases with congenital nasolacrimal duct obstruction. The age ranged from 12 to 36 months. They were examined by lacrimal endoscopy under general anesthesia.

Findings: All sites of lacrimal sac and nasolacrimal duct were filled with white purulent mass. The inner wall of 6 lacrimal sacs and all nasolacrimal duct appeared uneven. Site of nasolacrimal duct obstruction resisted manual opening in all the cases but one.

Conclusion: The observed endoscopic findings show why congenital nasolacrimal duct obstruction fail to resolve spontaneously in infants after 12 months of age.
