網膜中心静脈閉塞症に対するtPA網膜静脈内注入術の手術成績

Outcome of endovascular surgery with tPA injection for CRVO

要約　目的：網膜中心静脈閉塞症（CRVO）に対する組織型プラスミノゲン活性化因子（tPA）網膜静脈内注入術の治療成績を報告する。

対象と方法：過去19か月間でCRVOに対してtPA網膜静脈内注入術を行った12例12眼が対象で，男性8例，女性4例，平均年齢62歳であった。術前および最終受診時の視力（logMAR値）と中心窩網膜厚の変化を評価した。

結果：平均観察期間は8.5±5.1か月で，視力は術前1.01±0.51から最終受診時0.69±0.62と有意に改善した（p＝0.012）。中心窩網膜厚は，術前846μmから最終受診時307μmと有意に改善した（p＝0.0006）。術後合併症として1眼で遷延する硝子体出血，2眼で血管新生緑内障が生じた。

結論：CRVOに対するtPA網膜静脈内注入術は，視力改善と黄斑浮腫改善が期待できる。

Abstract　Purpose: To report outcomes of endovascular surgery with tissue plasminogen activating factor（tPA）injection for central retinal vazcular occlusion（CRVO）.

Methods: Twelve eyes of 12 patients（8 male and 4 female）who underwent endvascular surgery with tPA injection for CRVO. The mean age was 62 years. Changes in visual acuity（logMAR values）and central foveal thickness were evaluated.

Result: Mean visual acuity significantly improved from 1.01±0.51 before surgery to 0.69±0.62 at the final examination（p＝0.012）. Mean centaral foveal thickness was significantly reduced from 846μm before surgery to 307μm at the final examination（p＝0.0006）. One eye showed persistant vitreous hemorrhage and two eyes showed neovascular glaucoma.

Conclusion: Endovascular surgery with tPA injection for CRVO promises to improve visual acuity and reduce central foveal thickness.
