Iatrogenic Disease

1．「アンチピリン」中毒による小疱性角膜炎の1例

　75歳，女，脳出血，左半身不随で入院加療中39℃前後の不明熱を生じ，50％メチロン1.0cc 2筒，25％メチロン1.0cc 2筒を5日間にわたり注射したところ，突然全身状態の悪化をきたし，いわゆるショック症状を呈した。眼部は，眼瞼腫脹，結膜充血のほか，大小不同の水疱形成を多数両眼角膜に認めた。右眼は特にひどく一部びらん状であつた。抗生物質，副賢皮質ホルモン，ビタミンB₂剤の注射，内服およびタチオン点眼により数週間後には完全治癒をみた。

　根来（京都医大）：患者の既応，および病歴から考えまして，多分アンチピリン中毒によるものと考えられますが，アレルゲン・テストを行なつてみましたか。薬剤はハプテンであるため，従来のpatch testやscratchtestでは陽性率が非常に低いようですが，skin-windowtechniqueによりますと，陽性率が高くて，診断的に非常に価値があると思います（方法，J．Allergy,38；156，1966参照）。従来，開口部びらん性外胚葉症，Stevens—Johnsohn氏病と呼ばれていた，皮膚粘膜眼症候群の中に薬物アレルギーによつて起こされているものをときどき経験しますから，これらの疾患の場合，既往歴や現病歴をよく聞いて必要に応じて疑わしきものについて，アレルゲン・テストを行なつてみることが大切です。

A. Tamai (Matsue) opened with a report of a case of vesicular Keratitis induced by antipi-rin. A. Tamai (Matsue) and T. Motegi and Y. Yamagami (Toranomon) discussed the visual disturbance by newly developed secondary anti-tuberculosis medicines : Ebutol and Ethambutol. The former observed no change by ophthalmo-scopic and fluorographic examinations of the fundus but changes of ERG findings such as decrease of b wave and indistinctness of wavelets.
