A 65-year-old woman presented with decreased vision in both eyes. She was free of past history of ocular trauma or of heritable ocular disease. Her best corrected visual acuity was 20/30 right and finger counting left. Slitlamp examination showed splitting of the anterior iris stroma in several con-tiguous sectors bilaterally. Small fragments of the iris and pigment cells were floating in the anterior chamber of the left eye. Mature cataract was pres-ent in the left eye. Intraocular pressure was normalin each eye. Gonioscopically, the angle was extremely narrow at grade 1 in the left eye and wide in the right.

One month later, the patient returned with acute attack of angle closure glaucoma in her left eye. We performed peripheral iridectomy, extracapsular extraction and implantation of posterior chamber lens in the affected eye. Visual acuity improved to 20/25 with the intraocular level in normal range. There has been no clinically detectable progression of iridoschisis in either eye during the ensuing follow-up of 2 years.
