眼内レンズ二次移植術における屈折値を用いたレンズ度数決定法

Presurgical power calculations in secondary intraocular lens implantation

　無水晶体眼に対する眼内レンズ二次移植術において，他覚的屈折測定値を用いた方法（2種類）を含む5種類の眼内レンズパワー決定方法（A群：定数法，B群：RTEC　（Ray TracingError Correcting method）-refraction法，C群：SRK （Sanders-Retzlaff-Kraff）式，D群：Bink-horst式，E群：RTEC法）の精度を比較検討した．対象は前房レンズ（NOVA, Cooper-Vision社）を二次移植した無水晶体眼9例9眼である．眼内レンズパワーの誤差の平均値および標準偏差は，A群：+0.03±0.73ディオプター（D），B群：-0,37±0.71D,C群：-0.51±0.74D,D群：-1.49±0.73D,E群：-0,69±0.72Dであった（但し，＋は術後に遠視の状態を意味する）．誤差の平均値および標準偏差においては5群の間に有為な差は認められなかった．今回の結果から無水晶体眼に対する眼内レンズ二次移植術における眼内レンズパワーの決定方法として，他覚的屈折値を用いる方法（以下，レフ法）は眼軸長を用いる方法と比較して遜色なく，臨床的に使用しえると考えられた．眼軸長と屈折値という二つの違ったデータをもとに眼内レンズパワー決定を行えば，誤差をより小さくできる可能性があり，臨床的にも両者を使用すべきであると考えられた．

We evaluated the validity of five formulas for determination of refractive power of intraocular lens (IOL) implantation based on 9 aphakic eyes who underwent secondary IOL surgery. We tested 5 currently available formulas : A, simple refraction method, B, refraction method using ray-tracingerror correction (RTEC), C, Sanders - Retzlaff - Kraff (SRK) formula, D, Binkhorst's formula, and E, simple RTEC method.

When tested against the postoperative findings, there was no significant difference among the five formulas regarding the average and standard devia-tion of the errors. Further, the two refraction methods (A, B) gave similar results as other three formulas using the axial length in the calculations. It may advocated to use two different formulas to obtain more accurate presurgical estimates, prefer-ably using both refraction and biometric methods.
