帯状角膜変性症に対する角膜表層切除術の効果

Superficial keratectomy in band keratopathy

　帯状角膜変性症に対して角膜表層切除術を施行し長期間経過観察を行った．対象は3例5眼で，角膜表層切除術後2カ月，術後1年および最終観察時における表層切除された透明部分の面積を前眼部写真よりトレースし，ディジタイザーにて測定した．結果は全症例において術後の各時期における面積に有為な差は認められなかった．また表層切除部位における角膜上皮下混濁の再発も認められなかった．以上より原因疾患の消失した帯状角膜変性症に対する角膜表層切除術は，安全で長期的にも有効な術式であると考えられた．

We treated 5 eyes with band-shaped keratopathy with simple superficial keratectomy to remove the calcified deposits. During the postoperative follow-up, we measured the area of translucent central cornea from photographs processed by computer-assisted digitizer. We could conclude from the results that the central corneal area was the same at 2 and 12 months after surgery in all the 5 eyes. No recurrence of subepithelial opacity was obser-ved for 1 year at least.

While topical ethelene-diamine-tetraacetic acid (EDTA) is currently a preferred therapeutic modality for band keratopathy, the simple superfi-cial keratectomy is of value because of its ease and effect.
