後頭葉皮質下血腫による同名半盲の1症例

Homonymous hemianopsia developed in a case of occipital subcortical hematoma

　後頭葉皮質下血腫による同名半盲の1症例を経験した．症例は62歳の女性で激烈な頭痛を主訴として受診し，頭部CTにて右脳後頭葉全域と頭頂葉後部にわたる大きな皮質下血腫を発見された．出血は後大脳動脈の動脈硬化によるものと思われ，翌日血腫除去術を施行し，全身的には後遺症を残さずに治癒したが，眼科的には黄斑分割を伴う左側同名半盲が残った．

A 62-year-old female presented with severe head-ache with nausea and dizziness of abrupt onset. On computed tomography, a large subcortical hematoma was detected over the entire right oc-cipital lobe to spred to the posterior portion of the parietal lobe of the same hemisphere. The hemor-rhage was apparently consequent to arteriosclero-sis in the posterior cerebral artery. The hematoma was surgically removed the next day, followed by improvements in general conditions leaving no neurological defects. Left homonymous hemianop-sia with macular splitting persisted after surgery.
