成人T細胞白血病における眼症状

Ocular disorders associated with adult T-cell leukemia

　レトロウイルス感染による特異な白血病である成人T細胞白血病患者2例にみられた眼症状を報告した．症例1は，片眼に限局性の濃厚な黄白色の滲出物と血管の白鞘化，白線化を認めた．この壊死性網膜炎の原因として，サイトメガロウイルスの日和見感染が病理組織学的に確認された．症例2は，両眼に上強膜炎徴候がみられた．これらの眼症状は，全身的な免疫機能不全状態と関連して発生したものと考えられた．2症例は成人T細胞白血病ウイルスの濃厚汚染地域である南九州地区住民であった．

A 37-year-old male presented with acute necro-tizing retinitis involving the inferotemporal quad-rant of the right fundus. Another 65-year-old female developed bilateral acute episcleritis. Both were patients with adult T-cell leukemia caused by a retrovirus, human T-lymphotropic virus type-I. The ocular signs and symptoms concurred with thepatients' systemic immunosuppressed state involv-ing the T-cell function.

Both patients were resistents of Kagoshima Pre-fecture in southwestern region of Japan where the retrovirus is considerably prevalent among the population and occasionally causes recognizable leukemia/lymphoma. Ours may be the first descrip-tion of ocular complications associated with this recently defined retrovirus-induced disease.
