網膜中心静脈閉塞症を来したdural carotid-cavernous fistulaの1症例

Central retinal vein occlusion complicating dural carotid-cavernous fistula

　46歳女性のdural carotid-cavernousfistulaの症例において，眼球突出，球結膜充血，続発緑内障とともに，venous stasis retinopathyの発生と，網膜中心静脈閉塞症への移行が認められた．網膜出血，網膜浮腫の増悪に伴い，視力は低下したが，短絡血管に放射線学的な塞栓術（transvascular embolization）を行い，海綿静脈洞の圧を下げることにより，視力の上昇が得られた．また，眼球突出および続発緑内障の改善も認められた．

　Dural carotid-cavernous fistulaでは，網膜病変は軽度であり，これによる視力障害は少ないとされているが，本症例のごとく網膜中心静脈閉塞症を来すと視力の低下は著明となる．それゆえ，venous stasis retinopathyの段階であっても，増悪傾向がある際には，海綿静脈洞の減圧術を積極的に行うことが必要と考えられる．

A 46-year-old female presented with proptosis of the right eye and diplopia of 2 months' duration. Computerized tomography revealed pronounced dilatation of the superior ophthalmic vein and hypertrophy of extraocular muscles in both eyes. The right eye showed, ophthahnoscopically, venous stasis retinopathy. Three months later, central retinal vein occlusion developed with sudden loss of visual acuity.

Consequent carotid arteriography led to the diag-nosis of dural carotid cavernous fistula. Transvas-cular embolization of feeder vessels arising from the external carotid artery resulted in lessening of proptosis and increase of visual acuity.
