A 46-year-old female presented with proptosis of the right eye and diplopia of 2 months' duration. Computerized tomography revealed pronounced dilatation of the superior ophthalmic vein and hypertrophy of extraocular muscles in both eyes. The right eye showed, ophthahnoscopically, venous stasis retinopathy. Three months later, central retinal vein occlusion developed with sudden loss of visual acuity.

Consequent carotid arteriography led to the diag-nosis of dural carotid cavernous fistula. Transvas-cular embolization of feeder vessels arising from the external carotid artery resulted in lessening of proptosis and increase of visual acuity.
