網膜血管炎，視神経乳頭新生血管と硝子体出血を伴った地図状脈絡膜炎の症例

A case of geographic choroiditis with retinal vasculitis, disc neovascularization and vitreous hemorrhage

　網膜血管炎，視神経乳頭新生血管，硝子体出血を伴った地図状脈絡膜炎の49歳男性について報告した．両眼とも地図状脈絡膜炎に続いて網膜血管炎，網膜出血を来し，さらに，視神経乳頭新生血管，硝子体出血を起こした．硝子体出血を防止する目的で汎網膜アルゴンレーザー凝固を施行した左眼，施行しなかった右眼の両方とも黄斑部に網膜前増殖膜が形成され牽引性網膜剥離を来した．右眼には硝子体手術を施行したが，硝子体出血を繰り返すうちに黄斑部の網膜前増殖膜が再発した．

We observed a 49-year-old male with bilateralgeographic choroiditis associated by retinal vas-culitis, disc neovascularization and vitreous hemor-rhage over a period of 2 years. Vitreous hemor-rhage recurred repeatedly in both eyes. We could prevent further recurrence of vitreous hemorrhage in the left eye by panretinal photocoagulation with argon laser. Approximately one year after the patient was first seen by us, preretinal membrane developed over the macular region, followed by tractional retinal detachment. Pars plana vitrec-tomy in the right eye induced a temporal improve-ment in vision but did not prevent subsequent vitreous hemorrhages with further preretinal proli-feration.
