航空機墜落事故時の介達性外傷による視力障害の1例

Visual loss in a case with minor head trauma during aircraft crash

　1982年2月の羽田沖日航機墜落要故機に乗り合わせ，軽徴な頭部打撲後に左視力障害を来たした27歳男子の症例につき報告した。左眼底は網膜の小出血を2ヵ所に認めたのみであったが，受傷直後に施行した螢光眼底検査では網膜血管からの著明な色素漏出が認められ，特異な所見であった。墜落時の衝撃が介達外力として網脈絡膜・視神経の微小循環に異常をもたらしたことが原因と推定された。

A 27-year-old male was involved in a jetliner landing crash into the sea. He was only struck on his forehead by the front seatback without losing consciousness. Impairment of vision to 0.2 develop-ed immediately in the left eye. The fundus looked almost normal except two tiny retinal hemorrhages when examined 3 hours after the accident. Fluores-cein angiography showed profuse dye leakage from retinal capillaries and cystoid macular edema. The visual acuity in the left eye remained at 0.01 even after clearing of the cystoid macular edema.
