The posterior chamber lens (PCL) with plane modified J-loop occasionally develops iris capture as postsurgical complication. Its 10° angulated variety, developed for avoiding the capture, has its specific complication known as iris transillumination defect and microhyphema syndrome.

To avoid these complications, I designed a new modified J-loop PCL with 5° angulated lens haptic (HOYA J-5 PCL). It was implanted in consecutive series of 71 eyes in 70 patients treated in my institution.

All the treated eyes attained a postoperative visual acuity of 0.6 or more. Visual acuity of 1.0 was obtained in 54 out of the 71 eyes (76%). No serious complication was encountered during or following surgery except one case of posterior capsule rupture. In 38 eyes (54%), both loops were implanted and stayed in the bag. In 15 eyes (21%), one loop remained in the bag and the other out of bag. In 5 eyes (7%), both loops stayed out of the bag. The site of the loop could not be located in the other 13 eye.

There was no primary iris capture due to the anterior positioning of the lens optic. In 3 eyes (4%), secondary iris capture developed due to adhesion of the iris to the lens capsule or to the edge of the PCL.
