側頭動脈炎によると思われる両眼の急性脈絡膜虚血を来した症例

Bilateral acute choroidal ischemia due to temporal arteritis

　両眼の短後毛様動脈の閉塞により，後極部に広範囲の急性脈絡膜虚血を発生した1例を経験した．

　78歳の男性で両眼に急激に高度の視力低下を発生し，急性期には乳頭周囲から黄斑部を含む眼底後極部に，境界鮮明な網膜外層の灰白色の浮腫状混濁を認めた．螢光眼底造影により病変部は境界鮮明な脈絡膜螢光の高度の充盈遅延があり，そのため，造影早期には低螢光を示し，末期には過螢光を示した．本症の診断には螢光造影が有用であった．約3週間後には色素むらを示す軽度の網膜色素上皮の萎縮となった．視力は徐々に回復し，約9カ月後には両眼とも0.4にまで回復した．

　発病時，熱発，白血球増加，CRP強陽性，血清A／G比低下があったことから，原因として動脈硬化に側頭動脈炎様の血管炎が加わり発症したものと考えられ，両眼に発生した広範囲の短後毛様動脈閉塞による急性脈絡膜虚血と診断した．

A 78-year old male presented with sudden visual loss in both eyes. He had been under treatment for systemic hypertension for the past 18 years. Ile developed cereb-ral infarction with left hemiplegia at the age of 62.

Funcluscopy showed multiple areas of edematous opacity in the deeper retina in the posterior fundus in both eyes. Fluorescein angiography was characterized by filling delay and ensuing hyperfluorescence in the posterior choroid. Three weeks later, these changes turned into atrophy of the retinal pigment epithelium. The visual acuity recovered to 0.-1 in either eye.

During the acute phase of ocular involvement, we detected leukocvtosis, increased C reactive protein and a decrease in albumin/globulin ratio in the blood serum. We suspected temporal arteritis in addition to arterio-sclerosis as the causative factors of choroidal ischemia in this patient.
