Abstract 文献概要

　緑内障6例8眼に手術を行い，術前1か月以内と手術3～19か月後に眼圧日内変動を検索した。内訳は原発開放隅角緑内障5眼，Rieger症候群2眼，ぶどう膜炎による続発緑内障1眼である。4眼に線維柱帯切開術，4眼にマイトマイシンC併用の線維柱帯切除術を行った。全例で術前に緑内障治療薬を使用し，術後は使用しなかった。術後では，眼圧が終日下降し，日内変動幅が少なくなった。線維柱帯切除術群のほうが切開術群よりも眼圧下降効果が大きく，日内変動幅が小さかった。線維柱帯切除術群で最高眼圧が特に下降したことが日内変動幅が小さくなる一因であった。線維柱帯切開術群では，切除術群よりも眼圧下降効果と日内変動幅が小さくならず，日内変動リズムが残っていることが推定された。

　We evaluated the diurnal variation of interocular pressure（IOP）before and after surgery in 8 eyes of 6 patients of glaucoma. The series comprised primary open-angle glaucoma 5 eyes,Rieger syndrome 2 eyes,and uveitis with secondary glaucoma 1 eye. Four eyes received trabeculotomy and the other 4 eyes received trabeculectomy with adjunctive mitomycin C. All the eyes received medication for glaucoma before surgery and none after surgery. When evaluated 3 to 19months after surgery,IOP was decreased throughout the day with reduced diurnal variation. This effect was more pronounced in eyes after trabeculectomy than after trabeculotomy. Eyes receiving trabeculotomy showed lesser reductions in IOP and diurnal variation. It appeared that these eyes still retained the rhythm of diurnal variation to a certain extent.
