非定型的な眼底所見を呈した真性赤血球増加症の1例

Atypical fundus involvement in a case with polycythemia vera

　両眼の急激な視力障害で発症した真性赤血球増加症の1例について報告した。

　症例は49歳の男性で，網膜の浮腫状の混濁と網膜血管の変化をわずかに示すのみであった。螢光眼底撮影で，腕網膜循環時間と網膜内循環時間の軽度の遅延があり，黄斑部を含む広範な色素上皮の障害と脈絡膜の循環障害を示す所見を認めた。ERGとEOGにより深部網膜，網膜色素上皮，脈絡膜を中心とした病変が示唆された。対症療法による視機能の回復後，真性赤血球増加症と確定診断された。その後も本症の増悪・寛解に伴う視力の変動をみた。本症例は両眼にみられたことから，動脈の攣縮などの局所的要因でなく，血液の滞流が両眼の網脈絡膜におこったためと考えられる。

A 49-year-old male developed acute, bilateral, marked visual failure which later proved to be an unusual complication of polycythemia vera.

When seen by us on the 15th day after onset of visual failure, funduscopy showed mild retinal edema, partial retinal arterial narrowing and non-tortuous, slightly dilated retinal veins. The visual acuity was 0.03 right and 0.01 left. Fluorescein angiography showed a delay of arm-to-retina circulation time (16 seconds) and of intraretinal transit time. There were numerous areas of choroi-dal filling delay. The ERG showed slightly low-voltage a-wave and the EOG an decreased L/D ratio.
