虹彩異色性毛様体炎（Fuchs）の臨床像

Clinical feature of heterochromic cyclitis (Fuchs)

　北大眼科ぶどう膜炎外来を受診した虹彩異色性毛様体炎患者15例について，その臨床像を中心に報告する。症例の内訳は男性4例，女性11例であり，全例片眼性であった。角膜後面沈着物，著明な虹彩萎縮，虹彩後癒着を伴わない軽い虹彩炎，白内障，硝子体混濁は全例に認められ，6例（40％）に虹彩異色，4例（27％）に緑内障の合併がみられた。9例（60％）は0.1未満の視力であったが，その原因のほとんどは白内障であり，そのうち4眼に手術を行い，良好な視力を得た。虹彩炎に対してはステロイド剤は無効であり，緑内障に注意しつつ経過観察を続け，進行した白内障に対しては手術をするのが良い方法と考えられる。

Clinical features were evaluated on 15 patients with Fuchs' heterochromic cyclitis seen in our Uveitis Survey Clinic in the past 5 years. Four were males and 11 were females. The condition occurred unilaterally in all cases. All the eyes mani-fested keratic precipitates, marked iris atrophy, mild iritis without posterior synechia formation, cataract, and fine dust-like opacities in the ante-rior vitreous. Heterochromia was seen in 6 cases (40%) and secondary glaucoma developed in 4 (27%). The visual acuity was less than 0.1 in 9 cases and cataract was the main cause of the decreased visual acuity in most cases.
