エタンブトール中毒の眼科的副作用の統計的観察

Statistical studies on the ocular toxicity with ethambutol

　先に我々はEB中毒性視神経症前症例4例について報告したが，今回1979年4月から1981年7月までの2年3ヵ月間に，東邦大学付属大森病院限科外来を受診したEB服用患者100例を対象とし，経過観察を行った。EB中毒性視神経症と診断された患者は，外来48例中4例（8.3％），内科入院52例中10例（19.2％），計100例中14例（14％）である。発症群14例を対照群，すなわち対象例100例中発症群14例を除いた残り86例中から定期的に眼科的および内科的に各検査の施行された入院患者より19例を選び，これと比較検討し，統計的観察を行った結果，発症群と対照群において投与量および投与期間についてはDose-Response関係が必ずしも成り立たないこと，また眼科的副作用の発症と結核病巣の広がり，炎症所見，排菌の有無等，結核の重症度とは，あまり有意な関係は認められなかった。しかし，本症発症例において今回，赤血球数，ヘモグロビン1直，ヘマトクリット値が比較的低値を示し，有意差を認め，このことは本症発症に関字する一因子になりうると考えた。

We performed a prospective study of toxic optic neuritis in 100 patients under systemic ethambutol treatment during a 27-month period ending July 1981. Toxic optic neuritis developed in 14 out of the 100 patients (14%) comprising 4 out of 48 out-patients (8%) and 10 out of 52 inpatients. These 14 subjects who developed toxic optic neuritis were compared with 19 welldocumented inpatients who were under ethambutol treatment and who did not develop toxic optic neuritis.

There was no obvious correlation between the incidence and the dosage or period of ethambutol administration between the two groups.
