白血病性視神経症の1例

Leukemic optic neuropathy

　完全寛解状態にある急性リンパ性白血病症例に，髄膜浸潤が白血病性視神経症で発症した1例を経験し，寛解期の眼科検査の必要性を強調した。

（1）発症時，片眼性乳頭浮腫・後極部広範囲の刷毛状出血・静脈怒張を呈し，CT所見で患側視神経の肥厚をみた。2カ月後には，動脈の閉塞性変化・視神経乳頭および網膜の萎縮像を呈した。

（2）乳頭浮腫の発生は白血病細胞浸潤に由来し，視機能障害の原因は白血病細胞の視神経への浸潤に加えて，網膜の循環障害が関与していると結論した。

An 8-year-old boy developed acute unilateral visual loss during the remission of acute lymphocy-tic leukemia which had been under treatment for the past 13 months. Funduscopy revealed disc edema, widespread flame-shaped retinal hemor-rhages, venous dilatation and tortuosity in the af-fected eye. Swelling of the optic nerve was proven by CT-scanning. Pleocytosis and malignant cells were present in the cerebrospinal fluid (CSF), sug-gesting the infiltration of leukemic cells in the central nervous system.
