下斜筋過動を伴うV型外斜視の手術の量定について

Quantitation of surgical correction for V-exotropia with overaction of inferior oblique muscle

（1）下斜筋過動を伴い下斜筋過動以外の上下斜視，眼疾患，顔面骨奇形などを合併しないV型外斜視40例中，水平筋手術および下斜筋減弱術を併用し，V型が消失した31例について量定を検討した。

（2）1眼下斜筋減弱術によるeso shiftは，平均第一眼位で2.19°，上むき20°で4.32°であった。

（3）外直筋後転術の効果の平均は1.03°／mmであった。

（4）下斜筋減弱術を併用する場合の外直筋後転術の量定は，下斜筋減弱によるeso shiftを差し引き，残りの斜視角に対するおよそ1.5°／mmが適当である。

We evaluated the surgical outcome of 40 cases with V-exotropia with overacation of the inferior oblique in order to determine the optimum quanti-tation in this lype of strabismus. We defined the V-exotropia as manifesting deviation of more than 15,Δ or 8° between upward and downward gaze by 20 degrees each. In the present series, the amount of horizontal deviation ranged from 10° to 37° ave-raging 18.1°. The difference in deviation between upward and downward gaze ranged from 9° to 27° averaging 10.9°.
