増殖性糖尿病性網膜症に対する冷凍凝固術後に生じた特異な周辺部牽引性網膜剥離

Unusual peripheral traction detachment after cryoretinopexy for proliferative diabetic retinopathy

　周辺部より輪状に牽引性網膜剥離が発生したと考えられる糖尿病性網膜症6例6眼について検討を加えた。

（1）全例，剥離発生前に冷凍凝固術を受けていた。

（2）超音波像においても，周辺部に硝子体牽状物の陰影とともに網膜の挙上を認めた。

（3）術前および術中に，周辺部に円形ないし楕円形裂孔を3眼に認めた。

（4）治療は極めて困難で，視力予後は極めて不良であった。

（5）増殖性糖尿病性網膜症に対する盲目的な冷凍凝固は極めて危険であると考えられた。

Six cases of proliferative diabetic retinopathy with unusual peripheral traction detachment were reported. These cases had a past history of treat-ment of ophthalmoscopically unmonitored cryore-tinopexy. The characteristic findings were thick fib-rous proliferations with only peripherally elevated retina or round atrophic holes at the periphery of the fundus. Therapeutic attempts to reattach the retina were unsuccessful. Cryoretinopexy for pro-liferative diabetic retinopathy is not recommended unless good ophthalmoscopic control is available.
