緑内障を合併したRieger's anomalyの2例

Two cases of Rieger's anomaly with adultonset glaucoma

　壮年期に緑内障を発症したRieger's anomalyの2症例を報告した。

　症例1は35歳の女で，両眼同程度に，Schwalbe線の前方移動，幅広い線維柱帯，密生する虹彩突起とSchwalbe線に達する虹彩前葉などの隅角異常が観察された。眼圧上昇は左眼のみであったが，右眼にも眼圧上昇の既往が推測された。

　症例2は52歳の男で，両眼の著明なぶどう膜外反と，周辺虹彩前癒着が認められた。右眼のみに眼圧上昇をきたして，トラベクレクトミーを施行したが，術後1ヵ月頃より眼圧は再上昇した。

　以上の2症例から，Rieger's anomalyに合併する緑内障について，つぎのことが示唆された。すなわち，

（1）緑内障は，生涯のどの時期においても発症しうる。

（2）壮年期に緑内障を発症する症例のなかには．一時期眼圧が上昇した後，自然寛解の経過をとる場合がある。

（3）同一症例では，隅角所見の程度と眼圧上昇との間には密接な関係はない。

This paper describes two cases of Rieger's anomaly with adult-onset glaucoma.

A 35-year-old female developed conjunctival in-jection in the left eye. Gonioscopy showed an-teriorly displaced Schwalbe's line, widening of the trabeculum, numerous iris processes and insertion of the anterior stromal leaf of the iris to the Schwalbe's line in both eyes. While the intraocular pressure was elevated in the left eye only, nasal contraction of the visual field and early retinal nerve fiber defects were present in both eyes. The findings seemed to indicate that the intraocular pressure in the right eye had been elevated before.
