ジベカシン（DKB）点眼液の外眼部感染症に対する効果—日本におけるKoch-Weeks菌，Morax-Axenfeld菌感染の現況

The effect of dibekacin in the treatment of external ocular infection, with special reference to Koch-Weeks and Morax-Axenfeld infections of the eye

（1）113例の細菌感染によると思われる外眼部感染症患者を，0.3％ジベカシン（DKB）点眼液の1日4回点眼によって治療した。

（2） DKB点眼液の臨床効果は著効64例（57％），有効47例（42％），無効2例で，有効率は98％であった。すなわち，0.3％DKB点眼液は細菌性の各種外眼部感染症に対して，すぐれた点眼剤であることを示した。

（3）副作用は，治療の継続に支障のない軽微なものが4例，治療を中止したものが1例において認められたが，副作用に対して加療を要する程度のものは1例もなかった。

（4） Koch-Weeks菌による急性結膜炎は現在の日本においても急性結膜炎の中で重要な地位を占めており，全国的に四季を通じて発生している。その大部分は幼少年者の感染であり，その症状は古典的K-W菌感染の定型所見を呈さないため，臨床所見からは本菌の感染を推定できない。外眼部感染症の際の細菌検査の重要性を強調した。

（5） Morax-Axenfeld菌の感染も現在の日本において可成り症例がある。しかし現在本菌は古典的なBlepharoconjunctivitis angularisをおこすよりも，むしろ主として急性または亜急性の結膜炎の原因となっている疑いがある。

（6） K-W菌およびM-A菌感染に対してDKB点眼液は有効である。

A clinical trial of efficacy of 0.3% ophthalmic solution of Dibekacin (3', 4'-dideoxykanamycin B or DKB) on external bacterial infections of the eye in 6 clinical centers. A total of 113 cases were treated by instillation of the ophthalmic solution 4 times daily for 3 to 20 days. The treatment was judged as excellent in 64 of the 113 cases (57%) as the causative agents disappeared in 4 days and the cardinal synptoms in 7 days. The effect was good in 47 cases (42%) and fair or poor in 2 cases. Slight side effects developed in 5 cases. They did not necessitate discontinua-tion of the treatment in 4 of the 5 cases.
