Secondary keratoconus was observed in 10 eyes of 7 elderly persons over the age of fifty. Six cases were females. The condition appeared as posterior type of keratoconus (Itoi)1,2) and as oval cone (Perry)3) when seen by slitlamp micro-scope. The cone often attained a large size and was located in the inferior-temporal quadrant of the cornea.

As a particular feature of the past history of the patients, they had suffered from trachoma re-sulting in the formation of marked pannus and rupture of the Descemet's membrane. The dis-turbed state of corneal nutrition would have led to deformity of the cornea and posterior kerato-conus.
