角膜真菌症経過中に混合感染を起こしたと思われる1症例

A case of keratomycosis probably superinfected by bacteria

　73歳，男子の左眼に角膜病変が認められ，検査の結果，実質中に真菌の寄生が確認されたため抗真菌剤による治療を行い，当初は改善傾向を示していた症例が，突然潰瘍上側縁より実質中に濃い混濁が出現し急激に拡大して前房蓄膿を伴い角膜穿孔を起こした。これは真菌による初発病変に細菌による二次的な感染を起こしたものと考えられ，今後角膜真菌症を診た場合，細菌やウイルスによる混合感染に対しても十分注意を要すると思われた。

A 73-year-old male with corneal ulcer was diag-nosed as keratomycosis by microcopy of scraping of the lesion. After an initially favorable response to topical pimaricin and amphotericin B treatment, a new corneal opacity developed in the affected eye on the 7th day followed by hypopyon and perfora-tion of the cornea. The secondary lesion was ap-parently due to superinfection by bacterial organ-isms.
