緑内障に対するSclerangloreconstruction （強膜隅角再建術）

Sclerangloreconstruction in glaucomas

　Sclerangloreconstruction （以下SARと略）は緑内障に対する新しい手術である。この手術は角膜輪部で強膜を3層に剥離し血管に富んだ外層を前房隅角に挿入する手術で，技術的にはある程度の習熟を要する手術であるが，決して不可能な手術ではない。この手術の適応は，眼圧上昇が中等度までの各種の開放隅角緑内障（隅角が開放されており，炎症症状のない続発性緑内障を含む）で著しい限圧上昇を伴う慢性隅角閉塞緑内障には不適応であると考えられる。この手術は，他の緑内障手術と比較して合併症の少ない手術である。

　筆者は35眼にSARを行い，術後3ヵ月で，薬物療法なしに限圧が21〜25 mmHgの間にあったものが9眼，20mmHg以下が20眼，点眼薬を併用して20 mmHg以下のものが3眼，コントロールに失敗し他の予術を行ったものが3眼の成績を得た。

Sclerangloreconstruction (Fyodorov), a new type of glaucoma surgery, was performed in 35 glau-comatous eyes. The cases included chronic simple glaucoma (7 eyes), chronic angle closure glaucoma (4), developmental glaucoma (11) and secondary open angle glaucomas (13). Three months after surgery, the intraocular pressure was between 21 and 24 mmHg in 9 eyes, below 20 mmHg in 20 eyes, both without medication, and below 20mmHg in 3 eyes with medication. The intraocular pres-sure could not be controlled in 3 eyes with chronic angle closure glaucoma. No serious complications were encountered during or after surgery.
