眼科手術後にみられた過呼吸症候群

Hyperventilation syndrome after ophthalmic surgery

　18歳女子および19歳女子の局所麻酔手術術後にみられた過呼吸症候群について報告した。

　術前の十分な問診，前投薬の投与などにより患者の不安興奮を除去し，発作を未然に防ぐことが重要である。治療においては過呼吸症候群が神経性ショック，局所麻酔剤中毒等と類似の症状を示すため鑑別が必要である。

Two cases developed hyperventilation syn-drome 90 mimutes after ophthalmic surgery under local anesthesia. Both were females aged 18 and 19 years respectively. In the first case, blepharop-tosis was operated by levator resection. In the other case, exotropia was operated by lateral rectus recession.

As therapeutic measures, the former case was treated by intravenous injection of 10m1 of 8. 5% calcium gluconate and 10mg diazepam. The other was given 5m1 of 8.5% calcium gluconate and 20 mg diazepam. In spite of some controversy about the efficacy of calcium, the treatment was effective in both cases.
