1977年の急性出血性結膜炎小流行時の眼症状と潜在性神経障害

Ocular involvement and latent systemic neuropathy during an outbreak of acute hemorrhagic conjunctivitis in 1977

緒　言

　急性出血性結膜炎（以下AHCと略す）の臨床報告は1970年のガーナにおけるCharttcrjeeら^1）の発表以来世界各地から相次ぐ報告があり，病原菌ウイルスは甲野ら^2）によつてEntero-virus群の新しいタイプであることが分離固定され，神経合併症の発生が予測されていた。AHCの神経合併症としてRadiculomyelitisをはじめて注目したのは，インドでの大流行時のBharuchaら^3），Wadiaら^4）の報告である。以来台湾^5），セネガル^6），タイ^7）の各地からの報告が相次ぎ，神経合併症の頻度はAHC罹患者2万人に1人の割合と推定されている。幸い本邦では1971年の初流行以来今日まで未だ神経合併症の報告がないが，甲野^8）は九州で神経合併症の1例が確認されたと総説の中で述べている。AHCの神経合併症が将来本邦に出現する危険性は十分考えられるが，これに対しいまだ系統的な調査はなされておらず，わずかに共同研究者の丹羽ら^9）がAHC罹患後の不定愁訴を長期に観察した報告がある程度である。

　今回われわれは1977年に名古屋市および近郊で発生した。

A minor outbreak of acute hemorrhagic con-junctivitis (A.H.C.) occurred in the Nagoya area in April, 1977. We diagnosed and examined 24 cases with ocular signs and symptoms typical for A.H.C. The incidence and clinical features of the ocular involvement were similar to those in literature. We had an impression, though, that the clinical manifestations during the present outbreak were milder than those during the ini-tial pandemics in Japan in 1971 and 72 : the ocular pain and foreign body sensation were milder and the subconjunctival hemorrhage was less severe in our cases.
