流行性耳下腺炎に伴う角結膜炎

Ulcerous Keratitis as a Complication of Mumps

緒　言

　流行性耳下腺炎はウイルス（Paramyxovirus）による全身性感染症で，小児によくみられ，成人が罹患すると重篤な合併症を起こす。ウィルスは，耳下腺腫脹の起こる約1週間前より，10日後頃まで唾液などの分泌液中に認められる。したがつて，口，鼻，眼等の分泌物の飛沫などにより伝染する。潜伏期は約18日。ウイルスは感染後血中に出て，全身のあらゆる臓器を犯す，耳下腺などの唾液腺に炎症が必発する。その他の合併症は割り合いに少なく，その発生頻度順に列挙すると，睾丸炎，卵巣炎，膵臓炎，乳腺炎，甲状腺炎，前立腺炎，バルトリン腺炎，副睾丸炎，脳神経炎，髄膜炎，脳膜炎などがある^1〜3）。

　眼合併症は，涙腺炎^2，4），結膜炎^3），強膜炎^3，6），角膜炎^{2，6〜11，14，16）}，ブドウ膜炎^{2，12，13）}，視神経炎^2），網膜炎で^2），極く稀には眼筋マヒも起こす^1）。

A 5-year-old girl developed acute keratocon-junctivitis as a complication of mumps with photophobia, redness and lacrimation in her left eye. The condition became manifest 10 days after onset of parotitis. It developed into ulcerous keratitis with iritis and finally resulted in nubecula with visual acuity of 0.7. The right eye remained intact throughout.
