Varidase Buccalの眼科的応用

Ophthalmic Use of Varidase Buccal

　Varidase Buccalは非病原性溶血性連鎖球菌の産生する酵素のStreptokinase （以下SK）とStreptodornase （以下SD）との混合酵素製剤である。VaridaseはSKの持つ線維素溶解酵素活性作用とSDの持つデスオキシリボ核酸分解作用によつて創傷面の壊死物質並びに膿性分泌物を溶解することから，これらの除去のために従来局所的又は体腔内に注入して使用されていたものであるが，SKを静注或いは筋注によつて全身投与すれば，血中の線維素溶解能を増加せしめうることから，近年臨床的に線維素性炎症反応の緩解並びに血栓の溶解に利用されるようになつたものである。

　眼科領域におけるVaridaseの使用報告としては，Joseph M．　Miller et al （1957），　Paul Hur—witz （1959）等の報告がある。　Miller等は眼球を摘出した症例に抗生物質と併用して術後に起る浮腫の消槌に有効のことを記し，P．Hurwitzは網膜中心動脈塞栓，網膜中心静脈血栓，糖尿病性網膜症，慢出滲出性脈絡膜炎，虹彩毛様体炎，前房出血，硝子体出血，結膜下出血，皮下出血等41例に使用して好成績の得られたことを報告している。

27 patients with various kinds of eye disease were treated with Varidase Buccal (Lederle), 4 tablets daily for a period of 5 days. Following interesting results were obtained.

1) Of 8 patients with edema of the eyelid, three were of traumatic origin, one was after the acute attack of glaucoma, and two were following the surgical formation of upper lid fold. The lidedemas in all 6 cases were entirely absorbed 5 to 6 days after the beginning of the treatment. In other two cases, enucleation and exenteration of the eyeball were performed. Varidase was given a few days after the operation.
