時間と云う次元が加わつた視野変状—〔I〕中心性網膜炎の中心視野

　視野変状を考える場合，我々は狭窄或いは暗点としてその大小，形或いはその濃さ等を視野計上に画こうとするのが普通であるが，その画き始めから終りまでに時の隔りがあつても又あるのが普通であるが，そんな短い時間にはその境界は変らないと考えて，別にその時間的隔りを意にとめていない。これは暗点或いは狭窄は器質的変化の投映とみるからであろうが，私もあくまでそう考えたいがそうは考えられない種類の狭窄や暗点が数多く記されて来た。例えば血管暗点と称えられるものに於いて或いは緑内障に於て眼球に圧迫を加わえるとその暗点或いは盲斑が拡大するとされる。又或る疾患に於いて視野計の照度を明るくすると暗点が拡大するとされる。こんな暗点は一体何と考えられたのであらうか？その負荷となつた圧迫も照度もいくらでも多少することが出来るから暗点もそれに応じて，瞬間的には境界があるとしても，その境界がどんどん変るものとすれば，器質的変化はとてもこんなことではその大きさを変える筈はないから，この様な暗点は最早器質的変化の投映とは見徹し難い。その大小する範囲は負荷に対する機能的な変化と見なければならない，とすれば何もこの様な場合，暗点として境界を画く必要が果してあるであらうか？

The writer believes that scotoma or contractions of the visual field is the pathological organic changes of the retina manifested in the field of vision, and its boundaries are comparatively stable. If we follow this definition, functional changes such as Evans'angioscotoma should not be called "scotoma".

The writer observed, using campimeter, a mild spiral field, which is well known in hysteria, on the central visual field of the retina which had undergone chorioretinitis centralis. In his opinion, this spiral field is the manifestation of asthenia in the synapsis layer of the retina.
