先天性緑内障に対するCyclogoniotomy

Cyclogoniotomy on Congenital Glaucoma

　Barkan^1）6）やFrançois^7）は前房隅角に胎生期の中胚葉組織が異常に残存して，之を閉塞しているのが先天性緑内障の原因だと強調しており，従つてBarkanは，この隅角部の遺残中胚葉組織を切開し，房水とシユレンム氏管とを接触せしめるのが理論的に最も合理的な手術方法だとし，彼はゴニオスコープを用いるGoniotomyによつて，過去17年間に196眼中152眼に良好な成績を拳げている^4）。われわれはゴニオスコープを用いないで，毛様体剥離のときのように鞏膜側の切開創から隅角を解離する方法を試みた。このような方法は既にCusick （1954）^8）が報告しているが，彼は尖端がボーマン氏針に似たスパーテルを用いて行つており，このような鋭利なものでは不必要な部分を損傷する危険がある。

Gonioscopic findings of 3 cases of congenital glaucoma were described.

The authors conducted the following operation to allow access of aqueous to the trabeculae by stripping the embryonic mesodermal tissue remained inside of corneo-scleral trabeculae, that is, after incision and dissection of the bulbar conjunctiva as for cyclodialysis, the sclera was exposed and incised meridionally 3mm long at a distance of 2 to 3mm from the limbus and the spatula equipped with bilateral edge was introduced through the scleral opening directed toward the anterior chamber as cyclodialysis then by sweeping the spa-tula, the embryonic mesodermal tissue was stripped.
