トラコーマと緑内障—附 トラコーマ罹患眼の房水静脈

Trachoma And Glaucoma

　Cuénod，Nataf，MacCalla等の北アフリカ，近東アジアに於ける研究にては，トラコーマ罹患者に癒着性角膜白斑に因る続発性緑内障が多発する以外に，原発性緑内障も多発の傾向があり，トラコーマが原発性緑内障発生に対して何等かの好都合な影響を及ぼすのではなかろうかと指摘したが，私が大正7年から昭和29年の37年間に跨る期問から任意の5年を選んで，総受診患者について調査し，トラコーマ患者の原発性緑内障発生頻度と，非トラコーマ患者のその頻度とを統計的に比較したところ，総集計にてはやはりトラコーマ患者に原発性緑内障が多発するという結果になつたが，各年度の数字を点検すると，我国全般にトラコーマ蔓延度が高く然も現在に較べて亟症の多かつた古い年代(大正7，昭和3年)に於て，トラコーマ患者の原発性緑内障の頻度の方が寧ろ低く，又総患者数に対する原発性緑内障の頻度も，新しい年代よりも決して多くないという重大な矛盾に遭遇して，少くとも我国に於てはトラコーマが原発性緑内障を好発するとは云えないという結論を出した。

　続発性緑内障は統計上もトラコーマ患者に多発しておらず，殊に癒着性白斑を有するものは少数に過ぎなかつた。

　Nataf竝にその一派はトラコーマの原発性緑内障誘発機序としてMechanical theoryに立脚して，トラコーマ病変による角膜輪部附近の瘢痕化による房水流出阻害を想定しているが，之について批判を加えた。

Cuénod and Nataf (1928), MacCallan (1936), and Nataf (1952) expressed the opinion that secondary glaucoma should be properly caused dy adherent corneal leucoma among the patients with trachoma ; that also primary glaucoma tended to occur among them, and that trachoma might have a causative relation with the occurrence of primary glaucoma. Cuénod and Nataf considered that the cicatricial lesions of the corneal limbus and of its surrounding tissue, which were due to trachomatous affection, might be attributable.
